SJCON Title IX Anonymous Reporting Form

Reporting anonymously means you disclose that you believe you have experienced (or are aware of an incident involving) sexual harassment or sexual violence without identifying yourself or providing sufficient information to determine your identity or request any specific action. You may choose to make an anonymous report if you want someone in the College to be aware of your experience, but do not want to be involved in a formal investigation.

If you choose to remain anonymous you only have to complete the following required fields:

*Description of Incident
